The risk of perforation on insertion of a coil or a Mirena IUS is stated to be around 1 and 1,000 insertions. In some rare cases (fewer than 1 in 1,000 insertions) an IUS can perforate (make a hole in) the womb or the neck of the womb (cervix) when it is put in. This can lead to pain in the lower abdomen. If the doctor or nurse fitting the IUS is experienced, the risk of perforation should be extremely low. However, injury can occur if the device is not fitted correctly.
What information should I receive from my GP or the health clinic prior to insertion of the Mirena IUS?
The following information should be provided to all women regarding potential problems such as:-
• Back pain/discomfort – this is common but usually short lived and eased by analgesics and anti-inflammatory agents. Ongoing pain is rarely caused by Mirena.
• Bleeding – common for up to six weeks after insertion after insertion after which period they are very light and they stop altogether.
• Infection – especially if there has been a prior history of pelvic infections. Mirena does provide protection STD’s.
• Expulsion – this is rare but the highest risk is within the first day or so of insertion.
• Perforation – this is very rare.
• Pregnancy – the risk is approximately 1/1,000 Mirena users.
• Displacement of the device can indicate that the Mirena was not properly sited in the first place.
It is very important that patient’s are adequately informed of the risks of Mirena insertion. If the consent process is defective this can give rise to a legal claim for compensation for lack of informed consent.
The possible displacement of a Mirena device should also be investigated by a doctor in an appropriate and timely manner when it becomes clear that the coil threads cannot be detected. Proper referrals should also be made to the appropriate expert to establish what further medical steps need to be taken to locate the Mirena device and to remove it.
