The Medicines and Product Healthcare Regulatory Agency in the United Kingdom has issued a medical device alert for Action 3 Junior manual wheelchairs manufactured by Invacare. The medical device alert was issued on the 29th March 2011. The problem with the wheelchairs is that the push bar clamping brackets have failed resulting in a loss of control and the potential for serious injury to users or attendants. The manufacturer Invacare has issued a field safety notice but so far has been unable to trace all users especially where the push bar assemblies have been supplied as spare parts.
The MHRA has urged all those involved in the provision, use and maintenance of these wheelchairs, in particular, wheelchair service mangers, maintenance staff and contractors to do the following –
- Trace the affected wheelchairs
- Trace push bar assemblies supplied as spare parts before the 6th October 2010
- Obtain push bar assembly upgrade kits from Invacare and arrange for them to be fitted.
